There's no place like the comfort of home when you're recuperating from an illness or recovering from surgery. ComfortCare's specialized healthcare team can help make that possible . . . and with the reassurance that your every medical need is addressed.

ComfortCare's skilled nurses, occupational and physical therapists, speech and language dietitians, home health aides and social workers can come to your home and provide the personalized follow-up medical care you need.

ComfortCare requires a physician's referral and are covered by most insurance plans.

Chesapeake Regional's ComfortCare includes:

  • Skilled Nursing Services. Registered, licensed, bonded nurses can provide a full range of care requested by physicians including such services as managing medications, changing catheters, overseeing tube feedings, helping with ostomy care and handling injections.
  • Diabetes Services. Certified diabetes educators work with teams of professionals to help patients manage their diabetes at home. These healthcare practitioners assist with meal planning and nutritional guidance, insulin injections, blood glucose monitoring, lifestyle changes, pump maintenance, exercise plans, community resources referrals and other related care.
  • IV Therapy. This service often allows patients to go home sooner than expected after being hospitalized or prevents them from being hospitalized at all by bringing the skilled treatment and attention of medical professionals right into their homes. Services are available 24 hours a day and include total parenteral and enteral nutrition, IV antibiotics, fluid replacement, chemotherapy, pain management, blood and blood product transfusions and immunotherapy.
  • Rehabilitation Services. Rehabilitation therapists provide skilled, hands-on care in the comfortable surroundings of patients homes. Physical, occupational and speech therapists are specifically trained to handle rehab needs resulting from stroke, head injury, neurological diseases, arthritis, orthopedic surgery, bone fractures, amputation, heart attack, lower back and joint pain, and general overall weakness following illness.
  • Palliative Care Services. Palliative care aims to relieve suffering and improve the quality of life for the families and patients with advanced illness. Pain and symptom control is provided, as well as emotional support and counseling.
  • Pain Management Nursing Services. Certified pain management nurses work with referring physicians to help patients control their pain and achieve some comfort with minimal side effects. The service is appropriate for anyone who experiences chronic pain.

HomMed Health Monitoring System

Patients and their families can ensure in-home daily health monitoring through ComfortCare's partnership with HomMed L.L.C. The HomMed Health Monitoring System can be a benefit to those with chronic illness or other health conditions that require regular monitoring of heart rate, blood pressure, oxygen saturation, temperature and weight. The collected information is automatically transmitted to ComfortCare clinical staff, who reviews patient reports and responds appropriately to results.

Philips Lifeline Medical Alert Service

To help patients continue to live in the homes they love, Chesapeake Regional Medical Center offers Philips Lifeline, the nation's leading personal response and support service. Lifeline is an easy-to-use service that ensures that older adults living at home get quick assistance whenever it is needed 24 hours a day, 365 days a year. At the press of the personal help button, a certified Lifeline monitor responds within seconds with the appropriate help.

Philips Lifeline is available in all of Southside Hampton Roads and in North Carolina's Outer Banks.
