Surgery pre-registration

Patients must complete all demographic and insurance information before a procedure can be completed. The hospital verifies the information and complies with all insurance requirements prior to the day of surgery. It is the patient’s responsibility to be familiar with their insurance and to obtain authorization if required.

OB pre-registration

Patients must fill out the OB pre-registration form completely that is provided by the patient's obstetrician and mailed to the hospital as soon as possible. Scheduled outpatient diagnostic testing

For any scheduled diagnostic test, it is the patient's responsibility to be familiar with what their insurance company will cover and to obtain authorizations. An insurance card must be presented upon arriving at the hospital. Diagnostic tests cannot be completed without a written, signed physician order with an medically appropriate diagnosis. Patients must ascertain from their doctor how their orders are to be received by the hospital either via fax or by the patient when arriving at the hospital.
