Patients must complete all demographic and insurance information before a procedure can be completed. The hospital verifies the information and complies with all insurance requirements prior to the day of surgery. It is the patient’s responsibility to be familiar with his or her insurance and to obtain authorization if required. Upon admission to the hospital a copy of the insurance card(s) is required. OB pre-registration Patients must completely fill out the OB pre-registration form that is provided by the patient's obstetrician and mail it to the hospital as soon as possible. The name of the patient's obstetrician must be included on this form. Call 757-312-6508 for information on tours of the labor and delivery unit. Scheduled outpatient diagnostic testing For any scheduled diagnostic test, it is the patient's responsibility to be familiar with what his or her insurance company will cover and to obtain authorizations. An insurance card must be presented upon arriving at the hospital. Diagnostic tests cannot be completed without a written, signed physician order with a medically appropriate diagnosis. Patients must prearrange with their doctor how their orders are to be received by the hospital, either via fax or by the patient when arriving at the hospital.
